How to Use

Provide the customer satisfaction specialist with your name, membership number, and zip code. You will be provided with the name and telephone numbers of the participating hearing centers within your community.
Call the participating location to make an appointment.
Identify yourself as a member of the Beltone^® Network Program and use the code located in your membership booklet.
